Homeschool Record Keeping Help

I was wondering if anybody knows or has a sample of grading system or a sample/guide on how to use/do (evaluate) a class record. This is my very first and am very overwhelmed about the idea of record keeping and evaluating the scores.

I'm homeschooling my 1st grade child and wanted to keep records of her progress. People say it's not necessary, not until the 9th grade, but I still would really like to know. If anybody would be kind enough to share it and show it (an actual previous class record and teach me on how they did it), it is deeply and truly appreciated. Please help.
